Step-by-step instructions for completing the application form:
1. In the registration form, please provide your current mobile phone number and email address.
2. The application will be considered only if you agree to the collection, processing, and storage of personal data (on the last page of the Application) in accordance with the Law of Ukraine “On Personal Data Protection.”
3. All fields in the application are mandatory.
4. In the field “Do you or your household meet one or more of the criteria listed below? (You may select multiple options),” indicate only those criteria that you can confirm with supporting documents.
5. Attach supporting documents in all relevant sections of the Application.
6. In the field “Where is your household located?” please indicate the current address of the settlement where your household currently resides.
7. In the application, specify the number of each type of livestock and poultry you currently have (such as cows, pigs, chickens, goats, ducks, rabbits, etc.).
8. For the question “Please specify the required goods/services, as well as any additional costs (for example, delivery costs),” list separately the cost of the goods/services you plan to purchase with the assistance funds for maintaining livestock during winter (for example: grain | 20 kilograms | 100 UAH per kilogram).
9. In the field “Please specify when you expect to complete the planned works for insulating barns/shelters for livestock,” keep in mind that the assistance covers the period from November 2025 to March 2026.
